Client Consent & Agreement
I look forward to supporting your health journey. Before we begin, please take a moment to read this agreement so you know exactly how I work, how your information is protected, and what to expect from our sessions.
By booking a consultation, you confirm you have read and agree to the following:
________________________________________
1. Purpose of Naturopathic Care
Naturopathic care focuses on supporting your body’s natural capacity to restore balance using lifestyle guidance, wholefood nutrition, herbal medicine, and other natural approaches. It is complementary to, and not a replacement for, medical diagnosis or treatment from a qualified medical practitioner.
2. Sharing Your Health Information
I will ask for details about your medical history, current health concerns, medications, supplements, allergies, and lifestyle. This helps me provide safe, personalised care. Your information is confidential and protected by privacy laws.
3. Accuracy of Information
Please share complete and accurate details and let me know if anything changes during your care. Without this information, treatment may be less effective or, in rare cases, unsafe.
4. My Role and Scope
I do not diagnose medical conditions. If I believe your needs are outside naturopathy’s scope, I may recommend seeing another health professional.
5. Possible Risks
While generally safe, natural treatments may cause mild side effects or interact with medications. Please let me know straight away if you notice anything unexpected.
6. Appointment Changes
If you need to cancel or reschedule, please give at least 24 hours’ notice to avoid a cancellation fee.
7. Privacy and Legal Requirements
Your details will remain confidential, except where disclosure is required by law or to protect the safety of you, me, or others.
